A diesel particulate filter, or DPF, sits in your exhaust and traps the soot a diesel engine produces. Every so often the car runs a regeneration, burning that soot off at high temperature. On a healthy car this happens quietly in the background, but lots of short, cold journeys never get the filter hot enough, so it slowly blocks up.
